التعريف الطبي الفعلي للسمنة المرضية

هي حالة صحية خطيرة تنتج عن كتلة جسم مرتفعة بشكل غير طبيعي يتم تشخيصها من خلال وجود مؤشر كتلة الجسم (BMI) أكبر من 40 كجم / م²، نظرًا لأن مقدمي الخدمات الطبية ومرضاهم يفهمون كلمة “مرضي” بشكل مختلف، لذلك فإن المصطلح المفضل لم يعد “السمنة المرضية” ولكن يستخدم  بدلا منه “السمنة من الدرجة 3” أو “السمنة الشديدة” هذا يتيح لمقدمي الخدمات الطبية أن يبلغوا أن هذا المستوى من الوزن الزائد يمثل مشكلة صحية خطيرة دون أن يشعر المريض بخوف.

 

لماذا تعتبر  السمنة الزائدة مشكلة؟

يتيح إجراء تشخيص محدد لمقدمي الخدمات الطبية إعطاء الأولوية لمستوى المخاطر الصحية لمريضهم، كلما ارتفع مستوى الوزن الزائد، زاد خطر حدوث مشاكل صحية خطيرة، و وتزيد الإعاقة وتزيد مخاطر الوفاة المبكرة لهذا المريض، كما أنه يساعد على تحديد الأهلية لخيارات العلاج مثل جراحة فقدان الوزن أو الأدوية.

قائمة المشاكل الصحية ذات الصلة المحتملة للوزن الزائد والسمنة ، (وخاصة السمنة المفرطة أو الفئة 3) طويلة جدًا، بمرور الوقت، اكتشفنا أن العديد من الحالات الصحية ناتجة عن أو مرتبطة بالوزن الزائد، يمكن أن يساعد تقليل وزن الجسم على  تحسين  العديد من هذه المشاكل الصحية، فالسمنة المفرطة أو الشديدة أكثر من مشكلة في المظهر الجسدي، فهناك  مشاكل محددة تتعلق بالتمييز والتحيز الاجتماعي والاكتئاب والقلق وتدني احترام الذات.

فبعض المشاكل المرتبطة بالسمنة تأتي من الضغط على الجسم أثناء حمل الوزن الزائد مثل: ارتفاع ضغط الدم، وفشل القلب الاحتقاني، وتوقف التنفس أثناء النوم، وضيق التنفس، وآلام الأعصاب ، والتهاب المفاصل، وآلام الظهر، وحرقة في المعدة، وتورم الساقين، وتوسع الأوردة، والإعاقة الجسدية.

 

 

هل الإصابة بالسمنة الحادة أو المرضية تصنف على أنها إعاقة؟

الجواب القصير هو لا، على الرغم من أن العديد من الأشخاص الذين يعانون من السمنة المفرطة أو المرضية يمكن أن يكون لديهم ظروف صحية معيقة تنتج عن السمنة ويمكن أن يكونوا مؤهلين للحصول على إعانات العجز، إلا أن الحقيقة هي أن معظم الأشخاص الذين يعانون من السمنة المفرطة أو المرضية قادرون تمامًا على أداء وظائفهم الوظيفية، بينما المشاكل الأخرى تأتي من التغيرات غير الطبيعية في كيفية عمل الجسم بسبب وجود الدهون الزائدة في الجسم (الأنسجة الدهنية). أمثلة على ذلك:

كيف يكون علاج السمنة المرضية

أولاً يجب معرفة أسباب زيادة الوزن، من المؤكد أن الشخص يأكل أكثر من اللازم ولا يمارس التمارين الرياضية، ولكن هذا ليس سوى جزء من الصورة بأكملها، السمنة ناتجة عن الجينات التي حصلنا عليها من عائلتنا وتأثرت هذه الجينات ببيئتنا التي نشأنا فيها، بعض هذه التأثيرات تشمل عادات الأكل التي تعلمناها  وثقافتنا الغذائية ومجتمعنا وتاريخنا الطبي والتغيرات الهرمونية وأحداث الحياة المجهدة.

من المهم أيضًا معرفة أن السمنة مشكلة معقدة ومزمنة وليست ذاتية، لقد حاول الكثير من الناس لانقاص وزنه من قبل، بعضها ناجح لكن الغالبية تكافح من أجل إنقاص الوزن أو الحفاظ عليه، قد يكون من المحبط للغاية فقدان الوزن فقط لاستعادته مرة أخرى، لذلك من الأفضل  رؤية أخصائي طب السمنة هو المزود الطبي الذي درس أسباب والوقاية والعلاج من زيادة الوزن  والسمنة المفرطة أو المرضية، هؤلاء المختصون لديهم الوقت والتدريب وأدوات العلاج الشامل، ويشمل علاج السمنة المفرطة المرضية ما يلي:

  • تعديل النظام الغذائي: بناء على الأدلة الطبية.

  • التعديل السلوكي: للمساعدة في التغلب على العادات غير الصحية.
  • الدواء: إذا كان ذلك مناسبًا، كأداة للمساعدة في الرغبة في تناول الطعام.
  • جراحة إنقاص الوزن: إذا كان ذلك مناسبًا، لأولئك الذين يعانون من السمنة المفرطة أو المرضية (فقط عدد قليل من أخصائيي السمنة الذين يقومون بإجراء الجراحة، ولكن معظمهم يمكن أن يساعدوا الناس على الاستعداد بنجاح لجراحة إنقاص الوزن ومساعدة الأشخاص على الحصول على أفضل النتائج).
